For question number 3. Actually I haven't had a chance to flip thru the book that day. But I have the pdf version of the list of the drugs inside the Blue Book 1/2010 edition. From there, I just can type whatever drugs I wanna find but it only provides you with the generic name, code of the drugs as well as the category of prescriber.
